About Community Health IPA
Community Health IPA stands as a central hub for transforming healthcare delivery, leading the industry's transition to value-based care. Our unique model taps into the collaborative potential of community health centers, enabling the exchange of best practices in clinical care, the implementation of evidence-based change strategies, and the integration of a curated selection of vendor technologies to enhance in-person services.
What makes us different
What sets Community Health IPA apart from other IPAs is that all of our members are federally qualified health centers and, as such, they all share a common legacy and purpose: to provide high-quality primary and preventive services to all individuals in the community. Because we are exclusively governed by community health centers, all savings are reinvested directly into the health centers and the communities they serve, not investors or third parties.

Impact on Utilization
Transitions of Care for ED High Utilizers
In 2023, Community Health IPA implemented a transition of care program for high utilizers of emergency room services. Patients who had 3 or more ED visits in the preceding 6 months were contacted by a CHIPA RN when they had an additional visit to the ED.
The RN serves as a remote resource for care teams at CHIPA health centers to conduct outreach, coordinate visits pertinent to the hospitalization, assist with developing goals and tracking outcomes, and, when appropriate, collaborate with internal and external resources to escalate care.
The results have shown lower ED visits and lower avoidable ED visits.

In 2024 our CHIPA RN will serve more ED high utilizers and add in-patient discharges to the scope of services
coding accuracy
An increased focus on coding will roll out in 2024, to ensure codes assigned to diagnoses and procedures match the actual services provided and the medical conditions of the patients
